    <description>Yup its not just the US that&#39;s planning to radically reform the way it provides health care for its people.  Now its the turn of the NHS.

The new coalition government has launched its iniative Equity and Excellence: Liberating the NHS.

Reading through the paperwork I found it hard to cut through all the spin but the gist of it all seems to be that:-

1.  &lt;b&gt;Primary Care Trusts are to be abolished&lt;/b&gt; and groups of GPs will commission health care for their local communities.

Now I&#39;ve known a PCT commissioner or two and I can assure you they were highly skilled at negotiating contracts - do you think your doctor is?  Is that the best use of your doctor&#39;s time?  Can you see the possiblity here of kickbacks from private providers - or even NHS providers now that NHS providers are being forced to compete with each other?

Or is this just another back doorway to privatisation whereby the group of GPs pays a private company to do the commissioning for them?  I really don&#39;t know, it&#39;s very early days yet.

2.  I do like the idea of being able to &lt;b&gt;chose my GP&lt;/b&gt; as I see fit rather than being sent to one based solely on my postcode.  But am not really sure how useful it will be in practice.  Right now you have quite a bit of choice as most GPs are in group practices.  If you don&#39;t like one you don&#39;t need to move practice you just see another.  

The other advantage might be to see a GP at a different location.  It may seem useful to have a GP near work rather than home for instance - but just wait till you feel really sick  and don&#39;t want to travel miles for an appointment - you&#39;ll surely be wishing the doctor was around the corner.

The only real benefit I can see from this is that if you wanted to see a GP that combines their practice with say alternative or complementary methods or one where they had a great counselling serive.  In this case its definitely a reform worth having.

3.  &lt;b&gt;Getting rid of beuracracy.&lt;/b&gt;  Now this is a tricky one.  Everytime something goes wrong the public quite rightly demand that safeguards are put in place to prevent it happening again.  That requires people to work out a strategy, write up policy, train staff, make sure its working, check it regularly and continue to improve the system.  That takes people and those people are called beuracrats.  

I&#39;m not going to argue that all NHS management should be kept but be careful what you chop.  My preference would be to get rid of targets that make the government look good but in no way improve services to patients.  But I guess I&#39;m on a loser with that one.

Since my hip replacement in Oct 2009 I&#39;ve been experiencing quite a bit of pain in my knee so I used my six month post-op check up for that as well. - Not good!

On the hip front - the new SpR was very pleased with the implant.  In fact he said it could be used as the gold standard to aim for.  Everything in the right place and bone growing into the prosthesis along its entire length - Fab.  

Only problem is I&#39;m experiencing pain.  He&#39;s sent me off for blood tests as it might be infected (my worst outcome scenario) so keep you fingers crossed.  The thought of having it taken out is not one I&#39;m even going to consider right now.

My knee X-ray looked silly. There is a chunk of the top of the tibia missing.  Heavens knows where that went but I guess it somehow got mislaid following my rather serious rock-climbing accident in 2005.

The SpR&#39;s opinion?  Well without a moment&#39;s hesitation he said &quot;total knee replacement&quot; oh boy.  As usual I argued for alternatives but nothing else is going to work.  

My left knee is actually jumping the queue as the game plan had been to get my left hip done next but ho hum life goes on.

&quot;Sufficient until the day is the evil thereof&quot; is one of my favourite quotes - and I think I&#39;ll apply it to any further surgery.  Hang in there till the quality of life dictates its time.

And no, before you ask, there won&#39;t be a site called Knee Replacement and Recovery!</description>

    <description>When it comes to hip replacements a study published in the Annals of Rheumatic Diseases shows that obesity is more of a risk factor for men then for women.

Whilst there was no difference in the rate of hip replacements for obese women when compared to those of normal weight it was shown that obese men were 70 more likely to need a hip replacement than their normal weight counterparts.</description>
